Good Faith Estimate
Under the No Surprises Act, if you are uninsured or not using insurance, you have the right to receive a Good Faith Estimate of the expected cost of your care. Ask our team for a Good Faith Estimate before your appointment. If you receive a bill that is at least $400 more than your estimate, you may dispute the bill. Learn more at cms.gov/nosurprises.

What if Altus is out of network for my plan?
Many plans include out-of-network benefits. We can provide a superbill you can submit to your insurance for possible reimbursement, and we’ll help you understand your options.
Do you take UnitedHealthcare, Oscar or Baylor Scott & White?
Not yet — we’re in the process of joining these networks and they’re coming soon. In the meantime, you can use our self-pay rates or out-of-network benefits. Call us for the latest status.
Do you take Medicare or Medicaid?
Please call us to confirm current participation with Medicare, Medicaid and Medicare Advantage plans.
